Our practice area is Loughborough University Practice area (i.e. LE11 3TU).  However we are accepting registrations from Loughborough University students and their immediate families (i.e. husband/wife and children) provided they live within a three mile radius of the campus.  

All new students will receive a medical centre registration / health questionnaire form and an envelope addressed to 'The Chief Medical Officer' in their University joining pack.  

If you are intending to register at the medical centre, please return the form as follows:

UK Students

Please complete both sides of the form and sign it and post it to the medical centre in the envelope addressed to 'The Chief Medical Officer'

Overseas Students

Please complete both sides of the form and sign it but bring the form in person to the medical centre in order to complete the registration process.  Please note that you will only be eligible for free NHS care if you are on a University course which lasts longer than 6 months.

If you have not completed this form please ask at reception. New patients are also offered a new patient health check.

IMPORTANT NOTE
If you registered with us previously, for example last term, and haven't registered with another doctor between now and then, for example your home doctor, then you will still be registered with us and don't need to register again, but you do need to inform us of your new address
